Patients wishing to register at our Practice can do so using the link below, please check our catchment area first to ensure you live in the area we cover.

If you live outside of this area you should register with another GP practice closer to your home address.
Please register to the site you would prefer to collect medication from although you will still be able to attend either site. Please only complete the registration for one site.

If you wish to register for online services such as online ordering of medication, once registered please attend the surgery and bring along a form of photo ID such as passport, driving licence or other formal ID and we can set this up for you.
When registering children we encourage parents to provide a summary of their immunisation or a copy of the immunisation pages from their “red book” so that the practice can provide a timely and effective immunisation service.
If you are unable to fill out our online form above then please ask at reception for a paper copy, however it is a very simple form and if you are able to do this yourself it will really help us.
Healthy children under the age of 12 years do not need a new patient “check”, but we do like to have their red book brought in so we can ensure vaccination records are up to date. However, if you have a child under 12 that has significant health issues you should let us know.
The practice area covers approximately 400 square miles of rural south Lincolnshire covering approximately 50 villages. We have two surgeries;
Caythorpe Surgery, 52 High Street, Caythorpe, Grantham, Lincs, NG32 3DN and
Ancaster Surgery, 12 Ermine Street, Ancaster, Grantham, Lincs, NG32 3NP.
Both surgeries offer a full range of medical services.
If patients move outside of the practice area they are required to register with a practice in the area where they have moved to.
A recent government initiative allows practices to accept patients living beyond the defined practice boundaries but for home visits to be provided, by specific arrangement with NHS England, by a different practice. The Caythorpe & Ancaster Practice has however decided that we are not able to engage with this scheme as we feel that to do so would dilute, and potentially compromise the care that we provide to our registered patients living within the present practice boundary.
For geographical reasons patients living in the Caythorpe area tend to consult at Caythorpe Surgery and patients living in the Ancaster area at Ancaster Surgery. As our clinical records are stored on the practice IT system and are accessible on both sites of the practice there is the option of consulting at either site of the practice.
